Endoscopic Lift or SMAS: How to Achieve Natural Rejuvenation

As a plastic surgeon, I often hear patients ask: “Which is better — an endoscopic lift or a SMAS facelift? And does the so-called weekend lift actually work?” There is no universal answer, because every face has its own anatomy, degree of aging changes, and each patient has individual expectations.

What is an endoscopic lift?

An endoscopic lift is a minimally invasive technique in which a miniature camera (endoscope) and specialized instruments are inserted through small incisions hidden in the hair-bearing part of the scalp. The work is performed on the ligamentous structures of the face, not merely on displaced skin. This approach allows the surgeon to elevate the brows, smooth horizontal and vertical forehead wrinkles, soften nasolabial folds, and improve the contour of the midface.

Key features of the endoscopic lift include very small incisions hidden in the hair, reduced swelling and a shorter recovery time compared to traditional open techniques, focus on the upper and midface, and a natural, “non-tightened” result due to working with deep anatomical layers. This method is often suitable for patients with moderate age-related changes, typically between ages 35–45.

What is a SMAS facelift?

A SMAS facelift is a deep lifting procedure in which the surgeon works not only with the skin but also with the facial musculaponeurotic system (SMAS), which defines facial contours, the jawline, and the clarity of the lower face. This operation eliminates jowls, smooths pronounced nasolabial folds, and redefines the lower third of the face. The results are typically more dramatic and long-lasting.

This technique is most often chosen by patients with more advanced ptosis, usually after age 45–50, when endoscopic methods alone are no longer sufficient.

The “weekend lift”: what is it really?

This term often refers to less traumatic surgical interventions that allow the patient to return to normal activities within a few days. It may involve an endoscopic lift, a limited SMAS facelift, or combined minimally invasive techniques in patients with mild aging changes.

It is important to understand that a “weekend lift” is not a magic, no-downtime procedure, but still a full-fledged surgical operation that requires anesthesia, preparation, and postoperative supervision.

How to choose between an endoscopic lift and SMAS?

The final decision is always made together with the patient after an in-person consultation. During the examination, I assess the condition of the skin and underlying tissues, the severity of ptosis in the upper, mid, and lower face, the anatomical features including facial shape, brow position, and symmetry. Age, medical history, previous procedures, expectations, and readiness for recovery also play an important role.

In many cases, the best results come from a combined approach – an endoscopic lift of the upper and midface together with SMAS work along the jawline and neck. This strategy provides the most natural rejuvenation without creating the effect of a “new face.”

A natural result as the main goal

Modern plastic surgery aims not to simply “tighten” the skin, but to restore the harmonious proportions and volumes characteristic of the patient’s younger years. I always emphasize that a well-performed endoscopic or SMAS lift should not be obvious. Those around you may notice that you look fresher and more rested, but they should not immediately guess that surgery was done.

When should you see a surgeon?

If you notice brow descent, a tired or heavy expression, a blurred jawline, the appearance of jowls, or excess neck skin, it is time for a consultation. During the exam, I will determine whether an endoscopic lift is sufficient, whether a SMAS facelift is more appropriate, or if minimally invasive procedures are currently the best option.
