The chin and neck area is one of the first to reveal signs of aging. Loss of a well-defined contour, the appearance of a “double chin,” skin laxity, and platysmal sagging often contrast with an otherwise well-groomed face. This is why patients increasingly face a choice: to limit treatment to an energy-based device such as Morpheus8, or to proceed directly with a surgical neck lift. Let us examine the differences between these approaches to chin and neck contour correction, along with their possibilities and limitations.

Energy-Based Treatments for the Chin Area: The Capabilities of Morpheus8

Morpheus8 is an energy-based technology that combines microneedling with radiofrequency (RF) energy delivered at a controlled depth. The treatment is designed to stimulate collagen and elastin production, tighten the skin, and achieve moderate reduction of subcutaneous fat.

In the chin area, Morpheus8 is indicated in cases of:

  • mild to moderate skin laxity with minimal fat excess and no significant ptosis;
  • relatively young patients with preserved tissue tone;
  • use as a preventive or maintenance treatment following surgery.

The effects of Morpheus8 develop gradually over 2–4 months and typically require a series of treatments. The result is moderate skin tightening, slight contour improvement, and a fresher overall appearance. However, it is important to emphasize that energy-based treatments cannot eliminate significant excess skin, pronounced platysmal sagging, or a fully developed double chin. They improve tissue quality but do not change anatomical positioning.

Surgical Neck Lift: When Devices Are No Longer Effective

A surgical neck lift (neck lift, platysmaplasty, cervicoplasty) allows for a radical improvement of the chin and neck contours. During this procedure, the plastic surgeon works not only with the skin, but also with deeper structures, including the platysma muscle, subcutaneous fat, and in some cases even the mandibular angle.

When a patient presents with a pronounced double chin—which does not always correlate with excess body weight but is more often related to skin excess and sagging—a surgical neck lift is indicated. The procedure is also performed in cases of platysmal separation and weakening, as well as loss of a clearly defined cervicomental angle.

Consultation with a plastic surgeon is also necessary when there is little or no effect from energy-based or injectable treatments.

The main advantage of the surgical approach is the elimination of the underlying cause of unattractive chin and neck contours, rather than merely improving the quality of the soft tissues. After surgery, a clearly defined contour is achieved, neck anatomy is restored, and the result lasts for many years—on average 7–10 years or longer, depending on lifestyle and genetic factors.

Comparison: Energy-Based vs. Invasive Methods

Energy-based treatments, including Morpheus8, are appealing due to minimal downtime, the absence of general anesthesia, and a lower entry threshold. However, their effect is limited and temporary, especially in patients over 40–45 years of age or those with pronounced age-related changes.

Surgical neck lift procedures require operative intervention and a recovery period, but they provide a more dramatic and long-lasting result, correction of deeper anatomical structures, and a high level of predictability.

From a clinical perspective, energy-based treatments are not an alternative to surgery when clear surgical indications are present. They may serve as an adjunct or a maintenance stage, but not as a replacement.

Conclusion

The choice between Morpheus8 and surgical neck lift depends on the initial condition of the tissues, the patient’s age, and their expectations. For minor changes and preventive management of soft tissue laxity, energy-based treatments may be appropriate. However, in cases of a pronounced double chin, skin sagging, and loss of a well-defined contour, plastic surgery provides the most effective and long-lasting results.

During an individual consultation, a plastic surgeon will help the patient determine the optimal solution for their specific case. Sometimes the best choice is not the “less invasive” option, but the one that truly addresses the problem rather than masking it.
