A tired appearance, drooping eyelids, and a “sad” or heavy expression of the eyes are among the most common reasons both women and men seek consultation with a plastic surgeon. Many patients believe that blepharoplasty alone is enough to refresh their look. In reality, in a significant number of cases the issue is not limited to the eyelids alone. Changes in the brow and forehead area play an equally important role.

To achieve a predictable and natural result, it is essential to understand the difference between blepharoplasty and a brow lift, as well as to recognize the advantages of a comprehensive approach.

Blepharoplasty: Surgical Correction of Drooping Eyelids

Blepharoplasty is a surgical procedure aimed at correcting the upper and/or lower eyelids. In the context of a “tired look,” it most often refers to upper blepharoplasty.

The primary goal of the operation is to remove excess skin, adjust fat pads when necessary, and create a clearer, lighter contour of the upper eyelid. Excess, overhanging skin frequently creates a sense of heaviness, reduces the visible portion of the eye, and makes the gaze appear older.

Blepharoplasty is effective when:

  • there is a clear excess of upper eyelid skin;
  • the natural eyelid crease is hidden by overhanging tissue;
  • the eyes appear overloaded even when the brows are in a relaxed position;
  • in some cases, skin drooping (ptosis) partially restricts the visual field.

At the same time, blepharoplasty has its limitations. If the main cause of a tired appearance is brow descent, removing eyelid skin alone will not produce the desired effect. The eyes may continue to look heavy, and sometimes there is a feeling that “too much skin was removed,” although the real issue is located higher.

That is why a thorough anatomical assessment before surgery is of crucial importance.

Brow Lift: Surgical Elevation of the Brows

A brow lift is a procedure designed to elevate the eyebrows and correct the position of the forehead. With age, the brows—especially their outer portions—tend to descend. This creates shadowing over the eyes, visually presses on the upper eyelid, and forms a sad or fatigued facial expression.

A special place among these techniques belongs to the endoscopic brow lift, a modern method performed through small incisions in the scalp using an endoscope. This approach allows the surgeon to elevate the brows into an anatomically correct position without visible scars and to adjust the activity of muscles that pull the brows downward and create a chronically tense expression. As a result, pressure on the upper eyelid is reduced.

A brow lift is indicated when:

  • drooping in the eye area is more pronounced at the outer corners;
  • the patient constantly raises the forehead to “open” the eyes;
  • there is brow asymmetry affecting the perception of the gaze;
  • previous blepharoplasty did not produce the expected result.

It is important to understand that even with minimal excess eyelid skin, elevating the brows can significantly “open” the eyes without aggressive intervention directly on the eyelids.

Why a Comprehensive Approach Works Better

In most patients, a tired look is the result of a combination of factors: excess eyelid skin and brow descent. This is why choosing between blepharoplasty or a brow lift is often an artificial dilemma.

A comprehensive approach makes it possible to restore the correct architecture of the upper third of the face without an over-tightened appearance. This allows for a more natural and longer-lasting result while preserving the patient’s individual facial features.

In practice, the process looks like this: first, the position of the brows is evaluated. If they are anatomically lowered, a brow lift is performed (either classic or endoscopic). Only after that can the surgeon accurately determine how much eyelid skin is truly excessive and requires correction. This strategy allows for precise, delicate work without removing unnecessary tissue.

Conclusion

Blepharoplasty and brow lift procedures (classic or endoscopic brow lift) are not competing but rather complementary operations. The former addresses excess tissue in the eyelids, while the latter eliminates the cause of downward pressure from above and restores the proper position of the brows.

A comprehensive, individualized approach delivers the best outcome: an open, refreshed gaze without loss of natural expression. Most importantly, the right decision always begins with a detailed consultation and an honest assessment of anatomy—not with a universal recommendation to “just have blepharoplasty.”
